Steps for NEW PATIENTS

  1. Select “New Patient Visit”

  2. Complete the Consultation Request form.

  3. I will be in touch via phone and if we move forward with an appointment, a tentative scheduled appointment will be made.

  4. You will complete all intake forms at least 24-hours prior to our appointment.

  5. A scheduled appointment will be made and if our appointment is via tele-video, you will be given the link to join the video appointment.
